This artwork is by Larry Todd, an illustrator for Last Gasp and others. The artwork appeared on the back cover of Skull #4 (titled as “Skull Comics” in earlier numbers). Skull #4 (“Special Issue: Lovecraft”) was printed in May of 1972.
